Terms of Service
These terms govern your use of CredVault. By creating an account, you agree to these terms.
1. Acceptance of Terms
By creating a CredVault account or using any CredVault service, you agree to these Terms of Service and our Privacy Policy. If you are using CredVault on behalf of an organization, you represent that you have authority to bind that organization to these terms.
These terms form a binding agreement between you ("you," "your") and CredVault, operated by Polsia ("CredVault," "we," "us," "our").
2. Account Responsibility
You are responsible for:
- Keeping your login credentials (email and password) confidential and secure
- All activity that occurs under your account — whether authorized by you or not
- Notifying us immediately if you suspect unauthorized access to your account
- Maintaining an active email address registered with CredVault to receive important notifications
Tip: Use a strong, unique password. CredVault stores passwords using one-way hashing — we cannot retrieve your password if you forget it, but we can reset it for you from your account settings.
3. Credential Accuracy
You are solely responsible for the accuracy of the credential data you upload to CredVault. CredVault stores and facilitates sharing of credential information — we do not independently verify the accuracy of uploaded credentials.
Specifically:
- You must ensure that credential numbers, issuing authorities, issue dates, and expiration dates are accurate and up to date
- You must upload valid, unexpired credentials —伪造 or falsified documents are strictly prohibited
- You must renew credentials before they expire and update your vault promptly after renewal
- Expiration alerts are a convenience feature — they are not a guarantee of timely notice. You are responsible for tracking your credential renewal deadlines independently
If a hospital or employer relies on a credential you uploaded, that decision and its consequences are between you and that organization. CredVault is not responsible for the accuracy of the data you upload.
4. Acceptable Use
CredVault is for legitimate physician credential management. The following uses are prohibited:
- Uploading false, forged, or fraudulent credentials
- Using CredVault to impersonate another medical professional
- Attempting to access another user's credentials without their explicit authorization
- Using automated tools, bots, or scraping tools against CredVault without authorization
- Attempting to interfere with the integrity or security of CredVault's systems
- Using CredVault in any way that violates applicable law or regulation
Violations: CredVault reserves the right to suspend or terminate accounts that violate these terms. We may also report serious violations to relevant licensing boards or law enforcement.
5. Service Availability
CredVault aims for high availability, but we don't guarantee 100% uptime. Scheduled maintenance, upgrades, and unforeseen outages occur.
SLA & Uptime
Individual subscription tiers (Starter, Professional, Enterprise) may have different uptime commitments. For Enterprise customers, specific uptime guarantees can be negotiated in a separate Service Level Agreement (SLA).
Expiration Alerts
Email notifications are sent based on configured thresholds (90 days, 30 days, 7 days, expiration). We aim to deliver these reliably, but email delivery depends on external factors (spam filters, DNS, recipient server availability). Expiration alerts are a convenience, not a guarantee. You are responsible for tracking your credential renewal schedule.
Data Availability
In the event of a service disruption, your credential data is stored redundantly and can be restored. We maintain daily backups of all user data.
6. Intellectual Property
You retain full ownership of all credential data you upload to CredVault. By uploading your credentials, you grant CredVault a limited license to store, process, and share that data as directed by you through the service.
CredVault's name, logo, design, and technology are owned by Polsia / CredVault. You may not use CredVault's branding, trademarks, or copyrighted material without written permission.
Agency/organization accounts retain ownership of their organizational data. CredVault retains no rights to your organization or provider data beyond the limited license to deliver the service.
7. Limitation of Liability
CredVault is provided "as is." We make no warranties beyond those explicitly stated in these terms.
To the fullest extent permitted by law:
- No medical advice: CredVault is a document storage and sharing platform — nothing on CredVault constitutes medical advice, legal advice, or professional licensing guidance
- No verification guarantee: CredVault does not guarantee that credentials uploaded to the platform are authentic, valid, or current
- No employment decisions: CredVault is not responsible for decisions made by hospitals, staffing agencies, or employers based on your credential data
- Liability cap: In any event, CredVault's total liability for any claim arising from your use of the service is limited to the greater of (a) the amount you paid us in the 12 months preceding the claim, or (b) $100 USD
Healthcare decisions are serious: Never rely on CredVault alone for credential compliance. Always maintain independent records and verify deadlines with licensing boards directly.
8. Termination
Termination by You
You may terminate your CredVault account at any time from your account settings. Upon termination:
- All credentials are permanently deleted
- All share links are immediately invalidated
- Your account is deactivated (can be reactivated within 30 days if you change your mind)
Suspension by CredVault
We may suspend or terminate your account if you violate these terms, engage in fraudulent activity, or fail to pay subscription fees. We will attempt to contact you before termination unless immediate action is required to protect the platform or other users.
Data After Termination
After account termination, your data is deleted within 30 days per our data retention schedule. Data export is available before termination.
9. Data Export
You own your data. Before deleting your account or in the event of suspension, you can request an export of your credential data:
- Per-credential export: View and copy individual credential details from your dashboard at any time
- Bulk export: Contact us at contact@mycredvault.com to request a full data export before account deletion
We will provide your credential data in a machine-readable format within 5 business days of a written request.
10. Changes to These Terms
We may update these terms from time to time to reflect changes in the service, new features, or legal requirements. When we make material changes:
- We will notify you via email to the address registered with your account
- Updated terms will be posted on this page with a revised "Last updated" date
- Continued use of CredVault after changes take effect constitutes acceptance of the updated terms
For minor changes (clarifications, formatting corrections), the updated terms apply immediately upon posting. Material changes take effect 30 days after notification.
11. Governing Law
These terms are governed by the laws of the State of California, USA, without regard to conflict of law principles. Any dispute arising from these terms or your use of CredVault will be resolved in the state or federal courts located in San Francisco County, California.
If you are an agency or organization outside the United States, local law may apply to your use of CredVault — you are responsible for compliance.
